HRT and prescription weight-loss medicines treat different problems. HRT is used for menopause symptoms and health needs, while Mounjaro, Wegovy and Foundayo are prescribed according to separate weight-management criteria.
They may still affect the same person’s symptoms, medicines and day-to-day wellbeing, which is why neither should be reviewed in isolation.
Can Mounjaro, Wegovy or Foundayo be used with HRT?
Often, yes. There is no blanket rule that HRT must be stopped before starting Mounjaro, Wegovy or Foundayo. However, the clinician should know exactly which oestrogen, progesterone or combined HRT you use, how it is taken, whether you still have a uterus, your bleeding pattern, contraception needs, other oral medicines and any other medical conditions.
Do not stop HRT because you are starting weight-loss treatment.
Sudden or unplanned changes may cause symptoms to return and may leave a combined HRT regimen incomplete.
Why does the clinician need to know about your HRT?
Mounjaro, Wegovy and Foundayo are incretin-based medicines that can affect stomach emptying. Gastrointestinal side effects such as vomiting or diarrhoea may also make oral medicines less reliable if they are not retained or absorbed normally. Foundayo is itself an oral medicine, and its UK product information advises clinicians to consider potential effects on the absorption of other oral medicines.
This does not mean every oral HRT product will stop working. Specialist menopause guidance recommends reviewing oral components of HRT when incretin-based treatment is started or increased, with particular attention to oral progesterone where reliable endometrial protection is important. Evidence is still evolving, so changes should be made on an individual clinical basis.
- the name, dose and route of your oestrogen;
- whether you use progesterone and how it is delivered;
- whether you have a Mirena or another intrauterine system;
- whether your HRT is sequential or continuous combined;
- your expected and recent bleeding pattern;
- whether HRT is being used alongside contraception;
- whether vomiting or diarrhoea has occurred.
Foundayo also has medicine-specific interaction advice beyond gastric emptying, so tell your prescriber about all prescription, over-the-counter and herbal medicines you take.
If you use Wegovy tablets, they also have specific administration instructions: they are taken on an empty stomach and you should wait at least 30 minutes before taking other oral medicines.
Oral HRT versus patches, gel or spray

Oral HRT
Tablets pass through the digestive system. Effects on stomach emptying, vomiting or diarrhoea may create absorption concerns that deserve review, especially when reliable progesterone exposure is important.

Transdermal HRT
Oestrogen patches, gels and sprays are absorbed through the skin and do not rely on the stomach or intestines in the same way.
Transdermal oestrogen is not automatically required solely because someone uses Mounjaro, Wegovy or Foundayo. It is often preferred in women with obesity or diabetes for wider clotting-risk reasons, but route decisions remain individual.
Do not change only one part of a combined HRT regimen.
If you have a uterus and use systemic oestrogen, you usually also need adequate progestogen to protect the womb lining.
Why oral progesterone needs particular attention
Progesterone or another progestogen protects the womb lining from the effects of systemic oestrogen in women who still have a uterus.
Because micronised progesterone such as Utrogestan is commonly taken by mouth, specialist guidance advises clinicians to review whether oral progestogen remains the most appropriate option when incretin-based treatment is started or increased. The evidence base is limited and continues to develop, particularly for newer treatments such as Foundayo.
A review may consider whether oral progesterone remains appropriate, whether symptoms or bleeding changed, whether a non-oral progestogen may be preferable, and whether additional monitoring or a temporary adjustment is needed.
This is not a self-management decision.
Do not double progesterone, change the route or alter the number of days you take it without specific prescribing advice.
Menopause symptoms should still be reviewed
Weight loss does not automatically resolve menopause symptoms, and starting Mounjaro, Wegovy or Foundayo does not replace an HRT review.
- hot flushes or night sweats returning;
- sleep becoming worse after previously improving;
- new or worsening mood symptoms;
- vaginal dryness, urinary symptoms or painful sex;
- unexpected fatigue or reduced ability to function;
- new bleeding or a change in your expected bleed pattern.

When symptoms overlap

These are common with weight-loss treatment but may also be mistaken for hormonal or dietary symptoms.

Low mood, anxiety and reduced appetite need context rather than being attributed automatically to one treatment.
A joined-up review helps avoid repeatedly increasing HRT for symptoms caused by another issue, or blaming weight-loss medication when menopause treatment needs attention.
What about bleeding changes?
Unscheduled bleeding can occur with HRT, especially after starting or changing a regimen. New bleeding after starting Mounjaro, Wegovy or Foundayo should still be assessed in the context of your HRT, adherence, weight change and individual risk factors.
- bleeding starts after a long period without bleeding;
- it is heavy, prolonged or increasing;
- it continues outside the expected settling period;
- it appears after missed progesterone or vomiting;
- it is accompanied by pelvic pain or other new symptoms.
Do not assume bleeding is a harmless medication effect.
The timing and pattern determine whether HRT adjustment, examination or further investigation is needed.
Contraception, pregnancy and fertility
HRT is not contraception. Pregnancy may still be possible during perimenopause, even when periods are irregular.
Mounjaro can reduce the effectiveness of oral contraception after treatment starts and after each dose increase. UK advice recommends using a barrier method or changing to a non-oral contraceptive method for four weeks after starting Mounjaro and for four weeks after every dose increase.
Foundayo may reduce the effectiveness of oral hormonal contraception. UK product information advises adding a barrier method or switching to a non-oral contraceptive method for 30 days after starting Foundayo and for 30 days after each dose increase.
Semaglutide (Wegovy) is not expected to reduce oral contraceptive effectiveness in the same way, although vomiting or severe diarrhoea can still interfere with pill absorption. Mounjaro, Wegovy and Foundayo should not be used during pregnancy. Current UK product information advises stopping semaglutide at least two months, tirzepatide at least one month and orforglipron at least three weeks before a planned pregnancy.
Do HRT and weight-loss treatment improve each other?
HRT should not be prescribed to make Mounjaro, Wegovy or Foundayo work better, and weight-loss treatment should not be used as a substitute for appropriate menopause care.
The practical benefit of reviewing both together is safer prescribing and better symptom management, not promising a combined weight-loss effect.
Do I still need HRT reviews while losing weight?
Yes. HRT should normally be reviewed regularly and sooner when symptoms, bleeding, medicines or health risks change.
Significant weight loss may improve blood pressure, blood glucose and other metabolic risks, but it does not mean HRT should automatically be reduced or stopped.

- whether the route and dose remain appropriate;
- whether progesterone protection is adequate;
- any bleeding changes;
- cardiovascular, clotting and breast-health risk factors;
- contraception and pregnancy plans;
