How to measure and apply Blissel

Key application steps

Wash and dry your hands before use.
Remove the tube cap. Before first use, check the seal is intact, then use the point in the cap to pierce it. Do not use a tube with a broken seal before first opening.
Insert the plunger fully into the applicator barrel, then screw the barrel onto the tube.
Squeeze the tube to fill the applicator to the marked dose line. This measures 1 g of gel.
Unscrew the applicator and replace the tube cap.
Lie down with your knees bent and apart. Gently insert the applicator into the vagina and slowly press the plunger fully down to release the gel. Do not force insertion if it is painful.
Remove the applicator and separate the plunger from the barrel.
Follow your pack leaflet: rinse a reusable barrel and plunger thoroughly with warm, clean water. If your pack has disposable barrels, discard the used barrel and rinse the reusable plunger. Do not reuse a disposable barrel.

Use Blissel exactly as prescribed and follow the leaflet supplied with your pack. Contact LIVVE if you are unsure which applicator you have or find insertion difficult.

What is Blissel?

A low-dose estriol gel used inside the vagina.

Blissel contains estriol, a form of oestrogen, in a water-based vaginal gel. It is used to treat symptoms of vaginal dryness, soreness and irritation caused by lower oestrogen after menopause.

One applicator filled to the mark contains 1 g of gel, providing 50 micrograms of estriol. Use the applicator supplied with your medicine and follow its instructions.

Blissel acts mainly in the vagina, with low absorption into the bloodstream. Local treatment still needs an individual assessment and regular review.

There is no single format that suits everyone. Your clinician will consider your symptoms, current HRT, preferences and medical history.

The first days

You may not notice a clear change immediately. Some local itching or irritation can occur when treatment begins. Contact LIVVE if it is severe or getting worse.
During the starting course

Use the prescribed daily dose for 3 weeks. Notice whether dryness and irritation begin to ease, and ask for advice if symptoms are not improving.
Maintenance treatment

The recommended maintenance schedule is one applicator dose twice weekly. Keep to your prescribed routine rather than adding extra doses when symptoms are troublesome.

How often is Blissel used?

Follow your dispensing label and the patient information leaflet. The recommended Blissel schedule is set out below.

Do not share your medicine or applicator.
Check with LIVVE before changing the dose or frequency.
Tell your clinician about any other vaginal medicines or treatments you use.
Store below 25°C and keep out of the sight and reach of children.

When to ask for a review

You have completed the starting course but symptoms are not beginning to improve.

You develop unusual discharge, pelvic pain or symptoms that could suggest an infection.

You develop any vaginal bleeding or spotting during treatment. Arrange prompt assessment rather than waiting for your routine review.

Suitability and safety

Local treatment still needs the right checks.

Tell your clinician about your personal and family medical history, current medicines and HRT before starting Blissel. The following conditions are listed as reasons not to use it in the product information.

Blissel is not intended for use during pregnancy or breastfeeding. It is not contraception. Vaginal infections should be treated before Blissel is started.

Current, previous or suspected breast cancer, or a known or suspected oestrogen-dependent cancer.
Unexplained genital bleeding or untreated thickening of the womb lining.
A current venous blood clot, a previous unexplained venous blood clot, or a known clotting disorder. Tell your clinician about any past clot.
Active or recent disease caused by blood clots in the arteries, such as angina or a heart attack.
Acute liver disease, or previous liver disease where liver function tests have not returned to normal.
Porphyria or an allergy to estriol or any ingredient.

Do not combine Blissel with systemic oestrogen.

The Blissel product information says it must not be combined with oestrogen used for systemic treatment because the safety and risks of the combination have not been studied. Tell LIVVE if you use an HRT patch, skin gel, spray or tablet so your clinician can choose a suitable vaginal treatment.

Report bleeding and other changes

Any vaginal bleeding or spotting during treatment needs assessment. Also report new breast changes, pelvic pain or unusual discharge. Your clinician may advise an examination or further tests.

Some other conditions need closer monitoring, including fibroids, endometriosis, high blood pressure, migraine, diabetes and heart or kidney problems. Read the leaflet for the full list and tell your clinician about your medical history.

A feeling of warmth or irritation can occur. Speak to LIVVE if it is severe, persistent or getting worse.
Pelvic pain

Pelvic discomfort is listed as uncommon. New or persistent pain should be assessed.
Genital rash or thrush

Genital rash and vaginal thrush are listed as uncommon.
Headache

Headache is listed as uncommon. Seek urgent advice for a new migraine-type headache.
Allergic reaction

The preservatives sodium methyl parahydroxybenzoate and sodium propyl parahydroxybenzoate can cause allergic reactions, sometimes delayed.

What is Blissel used for?

Blissel treats symptoms of vaginal atrophy caused by oestrogen deficiency after menopause, such as vaginal dryness and irritation.

What hormone does Blissel contain?

Blissel contains estriol. Each gram of gel contains 50 micrograms of estriol, and one measured applicator-dose delivers 1 g.

Is Blissel the same as Estriol 0.1% cream or Ovestin?

No. Both contain estriol, but Blissel is a 50 micrograms/g vaginal gel whereas Estriol 0.1% cream contains 1 mg/g. Their formulations, measured doses and licensed instructions differ, so they should not be compared dose-for-dose.

Is Blissel the same as Vagifem or Gina?

No. Blissel contains estriol in a vaginal gel. Vagifem and Gina contain estradiol in vaginal tablet form. Your clinician can help choose a format that fits your symptoms and treatment history.

Can Blissel be used with my oestrogen patch, gel, spray or tablets?

The Blissel product information states that it must not be combined with systemic oestrogen preparations because the safety and risks of combination treatment have not been studied. Tell your LIVVE clinician about all HRT you use.

Do I need progesterone with Blissel?

An additional progestogen is not routinely recommended for low-dose vaginal oestrogen when systemic exposure stays within the normal postmenopausal range. Your clinician will review your full medical and HRT history. This does not mean Blissel can be combined with systemic oestrogen.

How often is Blissel used?

The licensed schedule is one applicator-dose daily for 3 weeks, then one applicator-dose twice weekly as maintenance. Follow your own prescription.

How long does it take to work?

Improvement is gradual and varies between people. Contact LIVVE if symptoms are not beginning to improve after the starting course, or sooner if they worsen. Treatment continuation should be reviewed after 12 weeks.

How long does a 30 g tube of Blissel last?

A 30 g tube contains approximately 30 measured 1 g doses. How long it lasts depends on whether you are using it daily during the starting course or twice weekly for maintenance.

Can I switch to Blissel from another vaginal oestrogen?

A switch may be possible, but do not assume you need to restart the 3-week daily phase or copy the schedule from your previous treatment. Your clinician should confirm the appropriate Blissel schedule for you.

What if I miss a dose?

Use it when remembered unless it is more than 12 hours overdue. If it is, skip that dose and use the next dose at the normal time. Do not double the dose.

Can Blissel be used on the vulva?

The licensed instructions are for use inside the vagina with the dose-marked applicator. If you have significant external soreness or cannot use the applicator, speak to your prescriber rather than changing how you use it yourself.

What ingredients does Blissel contain?

What if I have bleeding or spotting?

Arrange prompt medical assessment for any vaginal bleeding or spotting during treatment. Do not assume it is simply due to dryness or the applicator.
