First 12 Weeks

What to Expect in the First 12 Weeks of Mounjaro, Wegovy or Foundayo

The first three months are a settling-in period. This guide covers Mounjaro injections, Wegovy injections, once-daily Wegovy tablets and once-daily Foundayo tablets. It is about learning how treatment affects your appetite, digestion, routine and wellbeing — not proving how quickly you can lose weight.

This guide explains what may happen during each four-week phase and when to contact LIVVE.

Starting Mounjaro, Wegovy or Foundayo can feel exciting and uncertain at the same time. It is common to wonder when hunger will change, whether side effects are normal and how quickly the scales should move.

There is no single week-by-week pattern. The overall principles apply to weekly Mounjaro and Wegovy injections and to daily Wegovy tablets, although the administration routine and dose pathway are different for each format.

What should I expect during the first 12 weeks?

Expect a gradual adjustment rather than an instant transformation. The early months are used to establish a safe routine, understand appetite changes, manage side effects, maintain food and fluid intake, and review whether the treatment and current dose remain suitable. Weight loss may begin during this period, but the first 12 weeks are not a race or a final test of success.

Yes — this guide includes both daily tablet options.

The same early priorities apply to Wegovy tablets and Foundayo: correct use, gradual adjustment, appetite and side-effect monitoring, adequate nutrition and clinician-led review. Their administration routines are different, so follow the instructions for the medicine you have been prescribed.

The first 12 weeks are not about reaching the highest dose or losing as much as possible. They are about finding a routine you can tolerate, making sure you can still eat and drink well, and building a treatment plan that is safe enough to continue.

What are the first 12 weeks for?

  • Adjust

    Learn how your body responds

    Notice appetite, fullness, digestion, hydration, energy and how treatment fits into daily life.

  • Protect

    Keep nutrition and fluids adequate

    The aim is not to suppress appetite so strongly that eating and drinking become difficult.

  • Review

    Make clinically guided decisions

    Side effects, appetite and progress help your clinician decide whether to continue, pause or adjust.

Your prescribed dose and treatment timing will depend on the medicine and your individual clinical plan. Follow the instructions supplied with your treatment and contact LIVVE before your next dose if you are unsure what to do.

How do Wegovy tablets and Foundayo fit into this guide?

Wegovy tablets and Foundayo are both taken once daily rather than weekly, but the first 12 weeks still serve the same broad purpose: helping your body adjust, establishing a reliable routine and checking that appetite changes, side effects and food intake remain manageable.

The practical routines are not the same. Wegovy tablets must be taken according to their fasting instructions, separately from food, drinks and other oral medicines. Foundayo can be taken at any time of day, with or without food, and does not use the same fasting routine. This article does not repeat full product-specific instructions, so follow the patient information for your prescribed medicine and contact LIVVE if missed doses, dosing errors or medicine-timing problems become frequent.

Foundayo and oral contraception: Foundayo may reduce the effectiveness of oral hormonal contraception. If this applies to you, use a non-oral method or add a barrier method for 30 days after starting Foundayo and for 30 days after each dose increase, following your prescriber’s advice.

A tablet is not automatically easier than an injection. The most suitable option is the one you can use correctly, tolerate and sustain.

Your first 12 weeks: a realistic guide

  • Weeks 1–4

    Starting and settling into a routine

    Your first phase is mainly about tolerance. Some people notice less hunger, earlier fullness or quieter thoughts about food within days. Others notice little at first.

    Mild nausea, constipation, diarrhoea, indigestion or tiredness can occur. Keep portions smaller, eat slowly and drink regularly. Large or rich meals may feel less comfortable.

    Choose a repeatable medication routine and follow the instructions for your prescribed injection or tablet. For Wegovy tablets, this includes the daily fasting and waiting period. Foundayo can be taken with or without food and does not have the same fasting routine. Do not repeat a dose or change timing without checking the patient leaflet or contacting LIVVE.

    A quiet first month is not failure. Starting treatment is intended to help your body adjust, not necessarily to produce the strongest possible effect immediately.

  • Weeks 5–8

    Building consistency and reviewing tolerance

    Appetite changes may become clearer, but individual responses still vary. You may notice smaller portions feel enough, fewer cravings, less snacking or less urgency around food.

    This phase is also a useful point to ask whether you are eating enough. Persistent nausea or very strong fullness can make protein, fibre and fluids difficult to maintain.

    Your clinician may discuss whether your current dose remains appropriate. A dose increase is not automatic and should not happen simply because the medicine-specific minimum interval has passed.

    • Review appetite: Is hunger manageable without making food unappealing?
    • Review symptoms: Are side effects settling, stable or worsening?
    • Review intake: Can you eat balanced meals and drink regularly?
    • Review routine: Are doses being used correctly and consistently?
  • Weeks 9–12

    Looking at progress and what is sustainable

    By this stage, you and your clinician should have more information about appetite response, side effects, adherence and early progress.

    A useful review looks beyond one scale reading. It may consider waist measurement, eating patterns, energy, movement, constipation, sleep, mood and whether treatment feels manageable.

    Some people will continue on the current plan. Others may need more time at the same dose, a clinically approved increase, a lower dose, a pause or a different approach.

    The next step should support safe continuation rather than chasing a particular dose by a particular week.

What may happen to appetite and “food noise”?

Food noise is an informal term for frequent or intrusive thoughts about food, eating or the next meal. Some people notice these thoughts become quieter, while others mainly experience earlier fullness or smaller portions.

  • less urgency to eat between meals;
  • fewer cravings or less interest in highly rewarding foods;
  • feeling satisfied after a smaller amount;
  • remembering meals later because hunger cues are quieter;
  • little obvious change during the starting phase.

The desired effect is more manageable appetite — not complete loss of interest in food. Contact LIVVE if you are regularly missing meals unintentionally or struggling to meet basic nutritional needs.

Contact LIVVE if treatment is causing significant anxiety around eating, making you afraid to eat, or worsening a previous difficult relationship with food. Weight-loss treatment should not make eating feel unsafe or unmanageable.

Practical eating and drinking basics

  • Nutrition

    Protein

    Include a high protein source at meals or snacks to support muscle and recovery while weight is changing.

  • Digestion

    Fibre

    Include fruit, vegetables, wholegrains or pulses as tolerated and increase gradually if constipation is an issue.

  • Hydration

    Fluids

    Drink regularly throughout the day, especially if appetite is low or you have vomiting, diarrhoea or constipation.

Smaller meals may feel more comfortable than large portions. Eat slowly and stop when comfortably satisfied. Very rich, greasy or large meals may worsen nausea, reflux or fullness.

Do not deliberately starve yourself because your appetite is lower.

Very low intake can worsen fatigue, constipation, dizziness, nutritional deficiencies and muscle loss.

Why the aim is not to race through doses

A higher dose is not a reward, and the maximum dose is not the treatment goal.

Dose progression exists to balance treatment effect with tolerability. Moving too quickly can increase digestive side effects and make it harder to eat, drink and continue safely.

  • appetite control is already useful;
  • weight is progressing appropriately;
  • side effects are still settling;
  • nutrition or hydration needs attention;
  • another medical issue or medicine needs review.

Dose increases follow medicine-specific minimum intervals, not deadlines.

Foundayo is taken at each dose for at least 30 days before any increase. Other treatments have their own dose-escalation schedules. Never increase early, use extra medicine or change strength without clinical approval.

How should progress be judged?

Weekly weight can move because of fluid, bowel habits, menstrual changes and the timing of meals. One flat or higher reading does not prove that treatment has stopped working.

  • overall weight trend rather than daily fluctuation;
  • waist or clothing changes;
  • appetite and food-noise changes;
  • ability to follow a consistent eating pattern;
  • energy, strength and activity;
  • blood pressure, glucose or other health measures where relevant;
  • whether the treatment feels tolerable and sustainable.

This article does not cover longer-term plateaus or maintenance in depth.

What should I keep track of before review?

You do not need to record everything every day. A few simple notes can make your review more useful:

  • your weekly weight trend rather than repeated daily weighing;
  • changes in hunger, fullness, cravings and food noise;
  • nausea, constipation, diarrhoea, reflux or abdominal discomfort;
  • whether you are managing regular meals, protein and fluids;
  • energy, sleep, mood and ability to stay active;
  • any missed doses, treatment errors or difficulties using the medicine;
  • questions you want answered before the next prescription or dose decision.

What is manageable, what needs review and what is urgent?

  • Often manageable

    Common early adjustment

    Reduced appetite, mild nausea, earlier fullness, mild constipation and small short-term scale fluctuations. These should not stop you eating, drinking or functioning normally.

  • Contact LIVVE

    Needs clinical review

    Persistent or worsening symptoms, struggling to eat enough, repeated vomiting or diarrhoea, worsening constipation, dizziness, reduced urination or uncertainty before the next dose.

  • Urgent help

    Do not wait for routine review

    Severe persistent abdominal pain, signs of serious dehydration, severe allergic symptoms, sudden or rapidly worsening vision changes, or severe constipation with pain, vomiting or inability to pass wind.

When should I contact LIVVE?

  • Symptoms

    Persistent side effects

    Symptoms are not settling or are becoming harder to manage.

  • Nutrition

    Poor intake

    You cannot eat reasonable amounts or keep fluids down.

  • Hydration

    Repeated vomiting or diarrhoea

    Especially with dizziness, dark urine or reduced urination.

  • Digestion

    Severe constipation

    Particularly with pain, vomiting, marked bloating or inability to pass wind.

  • Wellbeing

    Unexpected symptoms

    New mood changes, worsening health or symptoms affecting essential medicines.

  • Treatment

    Dose uncertainty

    You are unsure whether to continue, delay or change treatment.

Frequently asked questions

How quickly will I lose weight?

There is no reliable week-by-week figure for one individual. Some people lose weight in the first month, while others see little change until later. The first 12 weeks are mainly about tolerability, appetite, routine and building a safe treatment plan rather than reaching a final result.

What if I do not feel anything at first?

That can be normal. Starting doses are used to help your body adjust and may not produce a strong appetite effect immediately. Continue exactly as prescribed and discuss your response at review rather than increasing early or changing the dose yourself.

What if I feel too full to eat?

Use smaller meals or snacks, eat slowly and prioritise protein and other nutrient-dense foods. Keep drinking regularly. Contact LIVVE if fullness is persistent, you cannot meet basic food or fluid needs, you feel weak or dizzy, or weight is falling much faster than expected.

What should I eat in the first few weeks?

Keep meals simple and manageable. Include a protein source, some fibre-containing food and regular fluids. Very large, rich or high-fat meals may worsen nausea or fullness. You do not need a rigid meal plan, but eating as little as possible is not the goal.

When will my treatment be reviewed?

Reviews are arranged according to your LIVVE treatment plan and before clinically approved dose changes or repeat prescribing where required. Contact LIVVE sooner if side effects are persistent, eating or drinking is difficult, symptoms are worsening or you are unsure whether to continue.

Does this first-12-weeks guide include Wegovy tablets and Foundayo?

Yes. It covers Mounjaro injections, Wegovy injections, once-daily Wegovy tablets and once-daily Foundayo tablets. The appetite, nutrition, side-effect and review principles are similar, but the two tablets have different administration routines: Wegovy tablets require specific fasting and waiting instructions, while Foundayo can be taken with or without food.

Is Foundayo taken the same way as Wegovy tablets?

No. Both are once-daily GLP-1 tablets, but Wegovy tablets have a specific fasting and waiting routine. Foundayo can be taken at any time of day, with or without food. Follow the instructions supplied with the medicine you have been prescribed.
